All Approved Agencies

Meet with client to fill out forms. Please review the process with the client to see if they qualify.                          

Remember:  

  • Provide verification of Income & Identification.  ​

  • Fill in date of home visit.  Client will not be approved if this section is not completed.

  • Fill in checklist and send in with packet.

STEP  4

Client will receive a call from   DS to confirm ​Housekeeping class appointment.   

Upon completion of the Housekeeping class, a shopping and delivery date will be scheduled

 

We will only reschedule this appointment one time.

Shopping Day:

Monday 11:00-2:00

30 minutes to shop

Pickup Day :

Tuesday 11:00-2:00

Housekeeping Day

Wednesday 9:00- 12:00

 

Pick-up Day​​​​​

Please bring strong persons, tie downs, etc. to receive goods.   No substitutions will be made.

30 minutes to load.  If items do not fit, you must leave them.  No substitutions.

 

You may opt to use a Uhaul type vehicle for pickup. No vans or suvs.

After packet is approved​​​
